Medical form procedure – please do this ASAP as TC can take weeks to process this:

Fill it out like this:
Under Issue/re-issue of a student pilot permit – check Ultralight
Under Glider Ultra-light Aeroplane Issue/renewal of a pilot licence/permit – leave blank
Fill out personal info – legibly
Read questions about your health – Please do not add things such as seasonal allergies etc.
If you are healthy, leave it all blank.
Sign, date and have someone else sign as a witness (it can be anyone)
Parts C – leave it blank – if you can sign and certify above that you are healthy, you do not need to see a doctor.
Scan this or take a picture and email it to me at ppgontario@gmail.com
I will check it, and if all good then I will forward it to Transport Canada.

Once TC processes you form they will either email or mail you CAT 4 medical certificate that looks like this:
